Host: What types of questions are asked on a typical dog adoption form?

Michelle Otis: Well, besides the basic personal information, your full name, address, phone number, contact information, most adoption applications are going to want to know what your pet history is, what type of experience you have had with different animals and what animals you have had in the past or have at home right now . Most applications are going to want to know how many people in your household and what the characteristics are, how many kids, how many adults, what the ages are. Many applications are going to want to ask some questions about what types of behaviors are unacceptable to you and what you are looking for in a dog, lifestyle type questions so that they can make a better match.
