Linda Sherry: An identity theft affidavit is a special form developed by the Federal Trade Commission or the FTC to help you write down everything about your crime, have it on one center place. Get it note or write if you want to, but you don't have to and this is something that you can send to companies when credit is opened up in your name to prove your innocence.
