Host: What is involved in the application process?

Bob Ley: You will be asked pertinent information about yourself, in order to identify to the insurance company who you are. These questions will consist of, but not be limited to the following; full name and current residence address, date and place of birth, gender and social security number, medical information such as smoking habits, under medical care, any medical inflections and medications, hospitalization, family medical history etcetera. For most life insurance policies your licensed insurance agent will make arrangements for you to take a paramedical exam. A licensed will nurse will come to your residence or place of business and ask you more detailed medical questions, take a sample of your blood and or urine and in some cases collect your signed life insurance application and premium payment. The fluids will be sent to a registered laboratory for analysis and your application, medical history and premium payment will be sent to the insurance carrier.
